When applying for a place in St Laserian’s you will need to return the following:

1. Completed Application form signed by the parents or legal guardians;
2. Psychological Assessment report which fulfils the following criteria:

• Report dates from within the last 3 years
• Report confirms that the child has a Mild, Moderate or Severe and Profound General Learning Disability,
• Report recommends that a special school placement is required

3. Original birth certificate (this will be copied and returned)

Applications will only be considered where:

• the child has a Mild, Moderate or Severe and Profound General Learning Disability (as confirmed by an appropriate professional’s report included in the child’s application)
• the application form has been fully completed and signed
• the application includes all enclosures appropriate to the application.
